The PHS Transition Program is a specialized educational initiative catered to young adults aged 18-21. Our program is designed to provide comprehensive support and instruction to empower students in developing vocational skills, independent living skills, and functional academic skills. Through personalized goal-setting, hands-on experiences, and community engagement, we aim to guide our students towards independence and success as they transition into adulthood.
Key Components:
- Vocational Skills Development:
Our curriculum includes practical training and instruction in various vocational skills relevant to the interests and abilities of our students. From basic job readiness to specialized training in specific industries, we aim to equip students with the tools they need to pursue meaningful employment opportunities.
- Independent Living Skills:
We recognize the importance of fostering independence in daily living tasks. Our program emphasizes the development of essential life skills such as personal hygiene, meal preparation, household management, and financial literacy. Through hands-on activities and structured lessons, students gain confidence and competence in navigating the challenges of independent living.
- Functional Academics:
While academic achievement is important, we understand that traditional academic subjects may need to be tailored to meet the individual needs of our students. Our functional academics curriculum focuses on practical literacy, numeracy, and problem-solving skills that are directly applicable to real-life situations, ensuring that students are equipped with the knowledge and abilities they need to succeed in the modern world.
- Personal Goal Setting and Support:
Each student in our program is encouraged to set personal goals related to their academic, vocational, and personal development. Our dedicated team of educators and support staff provides individualized guidance, encouragement, and instruction to help students achieve their goals and reach their full potential.
- Community Engagement and Experiential Learning:
We believe that learning extends beyond the classroom. Students regularly participate in a variety of community-based activities, including job shadows, meetings with resource agencies, tours, community service projects, fundraising events, and field trips. These experiences not only provide valuable hands-on learning opportunities but also foster social skills, community integration, and personal growth.
- Fundraising and Enrichment Activities:
All proceeds from fundraising events organized by our program are directed towards the activity funds of the PHS Transition and ID programs. These funds enable us to provide students with enriching experiences such as weekly community outings and monthly field trips, allowing them to explore new interests, build relationships, and expand their horizons outside of the classroom.
The PHS Transition Program is committed to empowering young adults with disabilities to lead fulfilling and independent lives. By providing a supportive and inclusive learning environment, personalized instruction, and meaningful experiences, we aim to equip our students with the skills, confidence, and opportunities they need to thrive in adulthood and make positive contributions to their communities.
